1. Customer should take approval from AELB for radiation license.
2. Please submit application form(new or additional) with approval certification to APM nuclear technology sdn.bhd. (Refer to APM application form No: AOA 101).
3. Additional, changes or termination can be done anytime but you have to inform us 30 days before the effective date.
4. Purchase order should be sent to APM Nuclear Technology sdn.bhd.
5. Please make payment as below:
-Cheque/ bank draft/ postal order/local order (L.O)/ others to APM nuclear Technology sdn. Bhd. RM for payment and booking.Using the information below:

6. The used OSL badge should be returned to APM Nuclear Technology sdn bhd before 10th day of the following month using any courier service or poslaju service only.
7. The dose report will be sent to customer within 30 days.
